The Question

Interviewers ask this in several surface forms. All are the same question:

The phrasing shifts the burden slightly—"what do you know" is more openly a knowledge test—but every variant is asking the same underlying question: did you actually research us, and does what you found match what you say you want?

Why Programs Ask It

Programs have finite interview slots and rank-list positions. An offer extended to someone who declines is an offer that cannot go to someone who would have accepted. Program directors and coordinators track yield obsessively, and this question is one of their primary instruments for estimating it.

Three program-side concerns drive the question:

Understanding this framing matters: your goal is not to flatter. Your goal is to give the interviewer evidence that allows them to place you high on a rank list with confidence that you will accept.

What It Is Really Testing

The surface test is research effort. The deeper evaluation has three layers.

Specificity as a proxy for genuine interest. Anyone can say "great reputation." Only someone who actually read the program website, talked to current residents, or attended an information session can say something verifiably specific about a curriculum structure, a named research track, a call model, or a fellowship placement record. Interviewers have heard thousands of generic answers. A specific one is immediately distinguishable.

Self-awareness. The question also asks implicitly: do you know what you need from a training program? An applicant who can name what they want—procedural volume, continuity clinic structure, research protected time, a specific patient population—and then map those needs onto what this program actually offers demonstrates a level of professional self-knowledge that generic praise does not.

Communication precision under low stakes. This is an easy question with a known correct form. If the applicant cannot be specific and organized here, the interviewer has less confidence they will be specific and organized when the stakes are higher. It is a low-cost signal about how the applicant thinks and communicates in professional settings.

A secondary test, less often discussed: can the applicant say something positive and specific without being sycophantic? Flattery reads as low confidence or low candor. Specificity reads as competence.

Answer Architecture

Do not memorize a script. Build a transferable structure that you reload with program-specific content for each interview. The structure has three pillars.

Pillar 1: Name two or three specific, verifiable program attributes

These must be things you cannot say about every program. Generic attributes that fail this test: "diverse patient population," "strong didactics," "good fellowship placement." Attributes that pass the test because they require actual research:

Pillar 2: Connect each attribute to a concrete personal training goal or career need

The attribute alone is not enough. You must close the loop: why does that attribute matter to you specifically? This is where self-awareness enters the answer. The connection should be tight—not "I am interested in research" but "I want to develop a research focus in health disparities in cardiovascular outcomes, and the work coming out of your health equity track addresses that population directly."

Two or three tight connections beat five loose ones. More attributes without connections reads as a list, not an argument.

Pillar 3: Close with a mutual-fit statement that opens dialogue

End by positioning this as a two-way evaluation, not a sales pitch. A brief statement that acknowledges you are also trying to confirm fit—and that invites the interviewer to tell you more—accomplishes two things: it sounds like a professional rather than a supplicant, and it naturally extends the conversation in a direction you can handle. Something that functions like: "One thing I'd still like to understand better is how residents typically structure their elective time in the upper years" shifts the dynamic and shows you are still gathering information, not just performing enthusiasm.

Calibration notes

One Strong Worked Example

Context: Internal Medicine applicant interested in academic hospital medicine and health equity research, interviewing at a university program with a known health systems science track.

"Three things drew me here specifically. First, the health systems science curriculum integrated into the PGY-2 year—I read through the publicly available structure and it is one of the few programs that builds quality improvement methodology into the clinical year rather than treating it as an elective add-on. That matters to me because my research background is in care delivery for underinsured populations, and I want those skills embedded in how I practice, not appended after the fact. [Specific verifiable attribute + tight personal-goal connection. "Integrated into PGY-2" is not something you can say about every program.]

Second, I looked at where your residents have gone in hospital medicine fellowships over the past few years, and the placement pattern is strong in academic programs specifically—which is my target trajectory, not community HM. [Demonstrates actual research effort. "Fellowship placement" is generic; "academic HM specifically, verified via the website/coordinator data" is specific. States career target clearly.]

Third, a conversation I had with one of your current interns at a regional conference last spring gave me a ground-level picture of the call structure that matched what the program describes officially—that kind of consistency matters to me when I'm evaluating whether a program represents itself accurately. [Shows network research, not just website reading. The consistency point signals that the applicant evaluates programs critically—a professional posture, not a supplicant one.]

I do still want to understand how elective time in the third year is typically allocated—whether residents are driving those choices or whether it is fairly structured. That would help me see how the program fits a more focused subspecialty direction." [Mutual-fit close. Opens genuine dialogue. Positions applicant as still conducting due diligence, not performing enthusiasm. The specific question is answerable and relevant—not a trap question.]

What makes this work in aggregate: every claim is verifiable. No superlatives. Personal goals are named precisely. The close invites the interviewer to contribute information rather than just evaluate the applicant.

One Weak Example and Why It Fails

"I was really drawn to this program because of its strong reputation and excellent teaching faculty. I know you have a diverse patient population, which I think is really important for training, and the residents I've spoken to all seem very happy here. I think this would be a great environment for me to grow as a physician and I'm really excited about the possibility of training here." [Eighty-three words. Zero specific content.]

Failure analysis:

The cumulative effect of this answer: the interviewer has no new information about the applicant after hearing it. They cannot use it to place the applicant on a rank list with confidence. If they are estimating yield, this answer gives them nothing to work with, and the applicant's rank position suffers accordingly.

Follow-Up Traps

Interviewers regularly probe this question further. Each follow-up is an opportunity to deepen or undermine the impression created by the initial answer.

1. "Which rotation here interests you most, and why?"

What this is testing: Whether you know the curriculum in enough detail to have an actual preference, and whether that preference connects to your career goals.

Tactical response: Name a specific rotation or longitudinal experience—not "the ICU" generically, but a rotation you can describe with something you know about how it is structured here. Connect it directly to a training goal. If you have not researched the curriculum at this level, this follow-up will expose the thinness of your initial answer. Do the research before the interview.

2. "Have you spoken to any of our residents?"

What this is testing: Whether you invested enough to pursue primary-source information, and whether what you heard was positive enough to sustain your interest.

Tactical response: If yes, give one specific thing a resident told you that informed your view—without attributing sensitive comments by name if asked for discretion. If no, say so honestly and explain why (geography made the pre-interview dinner inaccessible, for example), and indicate what secondary sources you used. Do not fabricate conversations.

3. "How do we compare to other programs you're considering?"

What this is testing: Partly yield estimation again, partly how you handle a socially awkward comparison question without being evasive or disloyal to other programs.

Tactical response: You are not required to rank programs aloud and should not. A professional deflection that still provides information: describe what you are looking for across programs at a feature level, and explain how this program satisfies those criteria particularly well. You can acknowledge that other programs are strong in different dimensions without ranking them. Do not disparage other programs—the academic medicine community is smaller than it appears.

4. "What would you do if you matched here versus at your home institution?"

What this is testing: Whether your interest in this program is genuine or whether you are there to fill a quota, and whether geographic or institutional loyalty would override a match here.

Tactical response: Answer directly. If you would genuinely be excited to train here, say so and explain why using the same specifics from your opening answer. If your home institution is genuinely your first choice, this is not the moment to misrepresent that—interviewers are experienced at detecting performed enthusiasm, and a credible answer about what appeals to you here, even as a second-choice framing, is more useful than a false statement of preference. Honesty paired with specific reasons is more compelling than hollow enthusiasm.

5. "What concerns do you have about our program?"

What this is testing: Whether you have thought critically about the program, whether you can voice concerns professionally, and—importantly—whether you did enough research to have a real concern rather than a fabricated modest one.

Tactical response: This question rewards genuine engagement. A real, resolvable concern (geographic cost of living, a specific curriculum gap you noticed, uncertainty about elective flexibility) stated directly and followed by a genuine question inviting clarification reads as professionally mature. A fabricated "concern" that is obviously disguised praise ("my only concern is that I might love it too much") is transparent and wastes the moment. If you have no real concerns, say briefly that your research has not surfaced a specific one but that you are still in the process of learning and may have questions as the conversation continues.

Identity Variants

The core architecture applies to all applicants. These sections address where the answer requires substantive adjustment.

IMG applicants

The implicit risk for an IMG applicant in answering this question is that "why here" can read as "why the US"—a conflation that serves no one. The program is asking about their specific program, not about the applicant's immigration or training pathway. The answer architecture above applies in full: be specific about program attributes, connect them to training goals, close with mutual fit.

One adjustment: if geography is genuinely part of the story—an established connection to the city, a prior clinical rotation at this institution, family or community ties that make this location meaningful—name it explicitly and briefly, then move to program-specific reasons. Proactively addressing geography prevents the interviewer from wondering whether the program is a fallback or a geographic convenience. A geographic connection is not a liability; presenting it as the only reason to be there is.

IMGs who completed observerships or research at this specific institution should use those experiences as primary evidence of genuine prior engagement. This is concrete and not something every applicant can offer.

Visa-requiring applicants (J-1 / H-1B)

If your application requires visa sponsorship, the program already knows this from your application materials. You do not need to introduce it into your answer to "why this program" unless the program's visa sponsorship track record is itself one of your specific reasons—which is a legitimate attribute to name.

If visa sponsorship does come up in the context of this question, address it directly and without apology: acknowledge your visa status, note that you confirmed the program sponsors, and return immediately to clinical and training reasons. Dwelling on visa logistics in a "why us" answer shifts the frame from your training goals to your administrative status. Keep the proportion right.

Old grads and applicants with time gaps

This question can function as an asset for applicants returning after a gap if the answer architecture is used well. The gap itself—whatever its cause—has likely clarified what the applicant wants from training. That clarification can and should appear in the personal-goal-connection pillar of the answer.

A specific example of how this works: an applicant who spent time in clinical practice abroad or in a non-clinical role can use that experience to name a training need that the program specifically addresses—not generically, but because the experience revealed a gap the program can fill. This transforms potential vulnerability into demonstrated self-awareness. The interviewer hears not "I hope you'll overlook my gap" but "I know exactly what I need and I researched why your program provides it."

Do not over-explain the gap in the context of this question. If the question is "why this program," the answer is about the program. The gap explanation belongs elsewhere—in the personal statement, in a direct question about timeline, or in a brief framing sentence that does not take over the answer.

Applicants with a notable application feature (prior attempt, exam score, academic difficulty)

This question is one of the cleaner moments in an interview for an applicant who wants to redirect attention toward strengths. Because it asks prospectively about training goals rather than retrospectively about record, a specific, well-researched answer does exactly what the applicant needs: it demonstrates professional engagement, preparation, and goal clarity—none of which appear in a transcript or exam score.

The tactical principle: answer the question asked. Do not proactively introduce application features that the question does not require you to address. If the interviewer wants to discuss your record, they will ask directly. Use this question to establish a strong forward-looking picture. That picture then provides a frame for any retrospective questions that follow.

Couples match applicants

Geographic constraint is a real factor in couples matching, and interviewers often know or suspect it. The worst version of this answer is one where geography is clearly the primary driver but the applicant performs program-specific enthusiasm to mask it. Interviewers are experienced at detecting this, and a mismatch between stated enthusiasm and visible preparation damages credibility more than the honest version.

A workable approach: acknowledge the geographic dimension briefly and factually if it is clearly relevant (both partners are interviewing in this city), then spend the majority of the answer on genuine program-specific reasons. If you have done real research and found features you actually value, this is straightforward. If you have not—if the program is genuinely coverage geography and nothing else—then your research effort before the interview needs to go up, not your performance of enthusiasm. Programs that match couples match applicants can work with honest geographic reasoning paired with real program engagement; they cannot work with hollow enthusiasm that falls apart under follow-up.
